Take the time to do it now. The Advance Health Care Directive (AHCD) documents your health care wishes if you are no longer able to speak for yourself because of illness or accident.
Our workshop provides a step-by-step explanation on how to complete your AHCD.
-What to consider when choosing your Health Care Power of Attorney (Agent)-Make decisions on the kind of end-of-life care you want or do not want.
-If you already have a Living will, Health Care Power of Attorney, or AdvanceHealth Care Directive, learn how to review it to be sure it meets your current needs.
